Our bridge of care helps to prevent readmission within the critical 30 days of discharge through the management of skilled services and program designed for our client's and their family's unique needs. By bridging the gap between the hospital, and home, eliminating the need to transition to a skilled facility (nursing home) before returning home, we provide a pathway to recovery, renewed independence, and patient empowerment, which create an improved quality of life and better overall health. Some of the community programs we offer include coordination of a homemaker, caregiver and 'Meals on Wheels.' We also provide assistance obtaining and coordinating transportation, housing, and public aid.
